  • Request a discharge kit with samples for new patients
  • Sort and search to see your data your way
  • Make personal notes and observations about each patient
  • Maintain detailed profile information
  • Quickly access online records of recent patient activity including custom notes and discharge kit orders. Plus, Coloplast will support you and your patient throughout the entire process.
  • A Coloplast Consumer Care Advisor will confirm the order with you and follow-up with the patient. Order tracking information will be provided via e-mail for your convenience.
  • Your patient will receive the requested products along with the ostomy discharge kit within two business days.
  • A Consumer Care Advisor will contact your patient within one week to answer any questions and provide follow-up support.
  • Access to monthly wellness education addressing lifestyle issues.
